REMINDER!
The first time you log into your patient rewards account be sure to CHANGE YOUR PASSWORD. If you do not do this you will be locked out; the password provided to you is only a temporary one. If you become locked out, use the prompt under the box you would enter your password where it says, Forgot Username or Password? This will walk you through the process of resetting your password.
